Parking is available in our building, but it is not discounted.
Street parking is restricted. Valet parking stands are also available within 1-2 blocks.

Preparing for your appointment in advance will help us make your visit productive and efficient.
If you are new to this practice, you must complete a New Patient Medical History Form located in the What do I need for my visit section (accessible by button on this page) and bring in copies of your medical records, electronic or hard copies of your X-ray images and reports, and a list of all your medications, supplements and dosages (or bring in your pill bottles).
In order to insure we have the time to dedicate to the care that you deserve, we made the difficult decision to be “out of network” for Medicare, Medicaid and private insurances. We believe we can deliver the highest quality, personalized care at reasonable (and transparent) cost. Our office can show you how to submit your out-of-pocket expense to your insurance for any available reimbursement. Please see the FAQ section of this website to learn more.
